Job Summary

The Care Coordinator - Social Work performs various direct and indirect patient care activities, working closely with patients and families who are members of the La Rabida Medical Home Program. This role involves providing support for integrated care teams, maintaining external relationships with referring organizations, payers, vendors, and regulatory entities, and coordinating day-to-day care coordination and social work activities. The ideal candidate will possess excellent communication and interpersonal skills, demonstrated knowledge of service delivery systems and resources for children and families, and the ability to develop and implement plans of care.

Key Responsibilities

  • Perform various delegated direct and indirect patient care activities, including working directly with patients and families who are members of the La Rabida Medical Home Program.
  • Assess psychosocial needs and utilize interventions to enhance patient and family functioning and emotional well-being.
  • Provide telephonic or in-person/in-clinic crisis management as needed.
  • Coordinate and perform day-to-day care coordination activities, including providing patients with information, referrals, and assistance accessing community resources to address social determinants of health and other psychosocial/educational needs.
  • Collaborate with various providers and the Outpatient Clinic team to provide direct social work services for patients and their families, including assessment of needs, crisis intervention services, conflict resolution, and assessment of barriers to improve appointment compliance and engagement.
  • Initiate child welfare reports on behalf of providers/others within the Outpatient clinic and maintain follow-up with IL DCFS or other Children Services organizations.
  • Assist in maintaining external relationships with referring organizations, payers, vendors, and regulatory entities.

Requirements and Qualifications

Essential Qualifications:

  • Master's degree in Social Work required.
  • 3 years of work experience with children and families required.
  • Excellent communication and interpersonal skills.
  • Demonstrated knowledge of service delivery systems and resources for children and families.
  • Ability to develop and implement plans of care.
  • Organized and motivated self-starter, able to work independently while collaborating effectively as a team member with other healthcare professionals.
  • Experience in Microsoft Office, Word/Excel required.
  • Ability to communicate effectively with all.

Preferred Qualifications:

  • LCSW (Licensed Clinical Social Worker) preferred.
  • Previous care management/coordination experience in an ambulatory/hospital setting preferred.
  • Knowledge of medical social services and community services in the Chicago area preferred.
