About the Role: Hospice Patient Care Coordinator - Clinical Operations Specialist

We're seeking a compassionate and clinically skilled Hospice Patient Care Coordinator to join our team in a remote capacity. As a key member of our clinical operations team, you'll provide critical support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, safe hospice care services. Key Responsibilities:

  • Provide clinical and management support to the Clinical Nursing Supervisor, ensuring the day-to-day operation of the Hospice program.
  • Assist with admissions, nursing visits, and staffing as needed, to ensure seamless delivery of care.
  • Coordinate care with clients, families, caregivers, physicians, and other service providers to ensure comprehensive and patient-centered care.
  • Monitor patient records to ensure compliance with documentation requirements and program-specific rules.
  • Participate in quality assurance activities, including audits and data collection, to drive continuous improvement.
  • Provide orientation, training, and in-services to new hires and the care team, to ensure the quality and appropriateness of care.
  • Perform comprehensive nursing care to patients, including skilled nursing visits, coordination of care, and establishing plans of care.
  • Maintain accurate and thorough documentation, including admission through discharge, physician's orders, and changes in condition.

Requirements and Qualifications:

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse licensure (in good standing) in the state(s) of practice.
  • Current CPR certification.
  • Valid driver's license and automobile insurance.
  • Three (3) years of clinical experience, including:
    • Two (2) years of hospice experience.
    • One (1) year of supervisory experience in a clinical setting.
  • Business coursework (business, finance, accounting, or human resources) preferred.

  • Proficiency with EHR systems and their implementation/optimization in complex clinical settings.
  • Knowledge and comfort level working with technology and enterprise-wide support systems and tools.
