About the Role: Case Management Coordinator - Field Two Rivers Region

We're seeking a dedicated and compassionate Case Management Coordinator to join our Aetna Better Health team in the Two Rivers Region of Kentucky. As a Case Management Coordinator, you'll play a vital role in supporting the case management process, improving healthcare outcomes for our members, and providing advocacy, collaboration, coordination, support, and education through the use of care management tools and resources.

Key Responsibilities:

  • Evaluating Member Needs: Utilize care management assessments and data review to recommend approaches to resolving care needs, maintaining optimal health and well-being, and evaluating member benefit plans and available internal and external programs/services.
  • Identifying High-Risk Factors: Identify high-risk factors and service needs that may impact member outcomes and implement early and proactive support interventions.
  • Coordinating Care: Coordinate and implement Wellness care plan activities, monitor member care needs, and consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers to meeting goals and objectives.
  • Enhancing Medical Appropriateness and Quality of Care: Utilize negotiation skills to secure appropriate options and services necessary to meet member benefits and/or healthcare needs, and identify and escalate quality of care issues through established channels.
  • Empowering Members: Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices, and help members actively and knowledgeably participate with their providers in healthcare decision-making.
  • Monitoring and Evaluating Care: Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.

Essential Qualifications:

  • Experience: Minimum 6 months of experience in Foster Care and juvenile justice systems, Adoption Assistance, the delivery of Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services, or evidence-based practices applicable to the Kentucky SKY populations.
  • Professional Background: Minimum 3 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
  • Technical Skills: Minimum 1 year of experience in MS Office Suite applications, including Word and Excel.
  • Travel Requirements: Willing and able to travel 80-90% of the time to meet members face-to-face in the assigned region.
  • Residency Requirement: Must reside in or adjacent to the Two Rivers Region (Allen, Barren, Butler, Daviess, Edmonson, Hancock, Hart, Henderson, Logan, McLean, Metcalfe, Monroe, Simpson, Union, Warren, Webster, Ohio).

Preferred Qualifications:

  • Certification: Certified Social Worker (CSW).
  • Experience: Experience working with foster care and juvenile justice populations.

Education:

Bachelor's degree or a non-licensed master's level clinician in behavioral health or human services field.
