About the position
Responsibilities
• Contact patients with incidental and/or unexpected findings to offer follow-up options.
• Provide personalized nurse navigation interventions to patients requesting additional information or referrals.
• Assess patient educational levels and barriers to learning that may impede care.
• Ensure efficient use of provider time by recommending appropriate follow-up intervals and avoiding duplicate requests.
• Assist primary care providers (PCPs) in ensuring patients receive appropriate follow-up care.
• Obtain additional imaging only at the specific request of the PCP.
• Educate patients on the follow-up process and direct them to the correct provider.
• Utilize software programs to track daily and periodic assignments.
• Maintain communication with colleagues in the Incidental Finding program and Radiology and Oncology departments.
• Identify problems, offer solutions, and participate in their resolution.
Requirements
• Graduate of an accredited school of Nursing with a Bachelor of Science Degree in Nursing.
• Three to five years of progressive clinical nursing experience.
• Current license to practice as a Registered Nurse in the State of Michigan.
• Problem-solving skills and ability to initiate creative solutions.
• Demonstrated ability to navigate various software applications effectively.
• Effective communication skills with medical staff and other health providers.
• Working knowledge of Radiology procedures, protocols, and processes (preferred).
• Knowledge of Windows-based programs.
• Effective organizational skills.
• Personal presence characterized by honesty, integrity, and caring.
