Job Overview

We are seeking an experienced Hospice Patient Care Coordinator to join our team in Pocomoke City, MD. As a Patient Care Coordinator, you will provide clinical and management support to the Clinical Nursing Supervisor, ensuring the quality and safe delivery of hospice care services. You will be responsible for the day-to-day clinical operation of the Hospice program, assisting with staffing and scheduling, admissions, and nursing visits as needed. Key Responsibilities

  • Provide clinical and management support to the Clinical Nursing Supervisor to ensure quality and safe delivery of hospice care services
  • Assist in efficient and effective utilization management in accordance with client needs, physician orders, and payer requirements
  • Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers as necessary
  • Assure efficient coordination of caregivers through case conferences and scheduling process as assigned
  • Assist in monitoring patient records to ensure compliance with documentation requirements and program-specific rules for coverage and qualifying criteria
  • Assist in the QA process of medical records as delegated
  • Assume functions of Clinical Supervisor as required
  • Provide orientation, training, or in-services to new hires and the care team to assure the quality and appropriateness of care provided
  • Assist in the completion of annual competencies and on-site visits for clinical staff
  • Participate in and assist in the completion of monthly and quarterly audits
  • Facilitate the audit process for clinical and billing purposes to improve the efficient delivery of client care and to assure appropriate reimbursement for services rendered
  • Perform comprehensive nursing care to patients, including skilled nursing visits, coordination of care, and establishing plans of care

Essential Qualifications

  • Graduate of an accredited school of nursing
  • Current Registered Nurse licensure (in good standing) in the state(s) of practice
  • Current CPR certification
  • Valid driver's license and automobile insurance
  • Three (3) years of clinical experience required, consisting of a minimum of two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting

Preferred Qualifications

  • Business coursework (business, finance, accounting, or human resources) preferred in addition to a nursing license
  • Experience with EHR systems and their implementation/optimization in complex clinical settings
  • Proficiency with federal, state, and governing agency regulations
  • Participation in continued clinical and in-service education, as well as professional training required by law
