Job Overview

We are urgently seeking an experienced RN Case Manager to join our team in Sparta, WI. As a Field Care Manager Nurse 2, you will play a vital role in assessing and evaluating the needs of our members, guiding them towards optimal wellness, and facilitating interactions with resources that promote their care and well-being. Key Responsibilities

  • Conduct comprehensive assessments of members' needs and requirements to achieve and maintain optimal wellness
  • Develop and implement member care plans that address physical, environmental, and psycho-social health issues
  • Identify and resolve barriers that hinder effective care, ensuring that members receive the support and resources they need
  • Collaborate with healthcare providers, community resources, and family members to coordinate care and promote member well-being
  • Continuously monitor member care through assessments and evaluations, making adjustments to care plans as needed
  • Utilize electronic information applications and software programs, including electronic medical records, to document member information and track progress

Essential Qualifications

To be successful in this role, you will need:

  • A current Registered Nurse license with at least 2 years of experience in home case/care management
  • Experience working with the adult population, preferably in a home health or community-based setting
  • Knowledge of community health and social service agencies, as well as additional community resources
  • Ability to travel to members' residences within a 30-40 mile radius
  • Exceptional communication and interpersonal skills, with the ability to quickly build rapport with members and their families
  • Ability to work with minimal supervision, using independent judgment and decision-making skills
  • Intermediate to advanced computer skills, including experience with Microsoft Word, Outlook, and Excel
  • Excellent keyboard and web navigation skills, with the ability to learn new software applications

Preferred Qualifications

While not required, the following qualifications are preferred:

  • BSN degree
  • 3-5 years of experience in home assessment and care coordination
  • Experience with health promotion, coaching, and wellness
  • Previous managed care experience
  • Bilingual skills (English, Spanish)
  • Certification in Case Management
  • Motivational Interviewing Certification and/or knowledge

Compensation, Perks, and Benefits
