Job Summary

Our Licensed Clinical Social Worker APP position represents an innovative step forward in the relationship between primary care and behavioral health in our community. As an expert in Behavioral Health, you will direct and shape the integrated behavioral health services of the clinic in collaboration with the primary care team. Your primary responsibility will be to support the enhancement and expansion of our behavioral health programs by collaborating on patient care as part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and healthcare professionals. The ultimate goal is to improve healthcare quality, and you will be accountable for maintaining quality integrated behavioral health programs for patients and staff.

Key Responsibilities

  • Perform individual patient assessments of needs and develop patient-specific care plans collaboratively with the primary care team to ensure high-quality and well-rounded patient care.
  • Collaborate with providers and the care team to develop and implement structured, evidence-based behavioral health treatment as part of a coordinated treatment plan with a goal of improving quality outcomes.
  • Educate patients on resources available to them and conduct follow-up evaluations.
  • Work closely with the primary care team to understand the root cause of patients' psychosocial issues and implement effective treatment plans.
  • Support the enhancement and expansion of our behavioral health programs by collaborating on patient care as part of an interdisciplinary care team-based approach.

Essential Qualifications

To be successful in this role, you will need:

  • A Master's degree in Social Work.
  • Experience with psychosocial assessment, individual therapy, evidence-based counseling, and crisis intervention.
  • Experience working in the medical social work or community mental health field and knowledge of Integrated Behavioral Health in the community healthcare system.
  • Familiarity and comfort working with health-related issues, substance abuse, and history of trauma.
  • LCSW: Licensed Clinical Social Worker by the State of Oregon required upon start.
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association, preferred.
  • Certification through the National Association of Certified Social Workers and/or the Academy of Certified Social Workers, preferred.
  • Crisis Intervention Training and/or certification, preferred.

Preferred Qualifications

While not essential, the following qualifications are desirable:

  • Previous experience working in a hybrid on-site and remote care management setting.
  • Knowledge of electronic medical records and experience with digital communication platforms.
  • Experience working with diverse patient populations and ability to provide culturally sensitive care.
