Customer Service Quality 1 1. Demonstrate and promote CHAMP behaviors Provide excellent customer service to patients and other healthcare team members, with attention to the needs of all staff, with positive feedback from staff and co-workers 1 2. 1 3. Provide medication counseling to patients and families, as applicable to work area 1 4. Demonstrate effective oral and written communication skills 1 5. Demonstrate ability to escalate issues to management and/or collaborating prescribers as appropriate 2. Medication Management Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs 2 1. Provide medication therapy monitoring for assigned patients, adjusting medication therapy based on patient factors 2 2. Develop an effective interdisciplinary relationship with the health care team , with attendance medical rounds (where applicable) 2 3. Act as a source of medication information for the health care team, providing at least one documented educational inservice to nursing or provider staff 2 4. 2 5. Participate in patient-centered initiatives (e.g. antimicrobial stewardship, vaccines, etc.) 2 6. Ensure clinical continuity where applicable to work area 2 7. Consistently document interventions within the medical record 3. Medication Preparation and Distribution 3 1. Demonstrate competency in pharmacy automation and technology as applicable to work area Oversee sterile and non-sterile compounding to ensure compliance with regulatory statutes (e.g. USP Chapters), as applicable to work area 3 2. Oversee medication distribution/dispensing process in work area, to ensure safe, effective, timely medication distribution/dispensing 3 3. Demonstrate competency in and oversee controlled substance management, including monitoring and documentation 3 4. 4. Medication Safety, Regulatory Affairs, and Quality Assurance 4 1. Adhere to all departmental policies, procedures, protocols, and guidelines 4 2. Adhere to appropriate state and federal regulations 4 3. Report medication events and near misses 4 4. Demonstrate HRO principles and conduct in all interactions with patients and staff 4 5. Participate in quality assurance activities, including attendance at daily pharmacy safety huddle 5. Leadership Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members, via educational inservice or as a preceptor 5 1. 5 2. Participate in professional development activities 5 3. Participate in departmental and interdepartmental, and system projects and committees 5 4. Attend departmental staff meetings 5 5. Oversee pharmacy staff as necessary in work area 5 6. Participate in department or hospital-sponsored community service and volunteer events 5 7. Demonstrate membership within a local or national pharmacy society 6. Financial/Volume Results 6 1. Contribute to departmental cost-reduction/revenue generation 6 2. Participate in pharmacist-driven cost management protocols where applicable 6 3. Provide cost-effective clinical care (formulary stewardship) Qualifications EDUCATION (number of years and type required to perform the position duties): Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree EXPERIENCE (number of years and type required to meet an acceptable level of performance): Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required. Minimum of two years of relevant experience specific to practice area is required. SPECIAL SKILLS: This individual should demonstrate in depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or other areas of accountability): The individual in this position is held accountable to maintaining the highest of professional standards and promoting rational and cost-effective therapy to positively influence patient outcomes. Must meet Career Ladder criteria for Clinical Pharmacist II. Must complete all hospital and department competencies and requirements for employment and maintain annual continuing education credits as mandated by the State of Connecticut Department of Consumer Protection.
