Outpatient Clinical Pharmacist - Remote
About the position
Responsibilities
• Exhibits behavior and communication skills demonstrating commitment to high levels of customer service.
• Executes disease state management programs targeting chronic conditions such as anticoagulation therapy, heart failure, diabetes, hypertension, and COPD.
• Performs post-hospital medication reconciliation, identifying and resolving medication-related problems and coordinating patient care post-hospitalization.
• Manages, assesses, and recommends evidence-based medication therapy based on the patient's clinical needs.
• Analyzes medication history to formulate clinically sound recommendations through medication reconciliation.
• Demonstrates appropriate judgment on conditions requiring consultation or referral to the patient's PCP or other healthcare professionals.
• Educates patients and families about preventative care, medical issues, and prescribed medications.
• Addresses new patient referrals, labs, tasks, patient follow-up, and drug information questions in a timely manner.
• Documents all aspects of patient care and completes required paperwork for billing and compliance.
• Utilizes pharmacy data and reports to develop and implement strategies to improve quality measures.
• Maintains productivity standards through effective multi-tasking.
• Participates in professional development activities related to chronic disease management and quality initiatives.
• Assists in clinical instruction and supervision of medical assistants, pharmacy technicians, and students.
• Uses, protects, and discloses patients' protected health information in accordance with HIPAA standards.
• Performs additional duties as assigned.
Requirements
• Current Oregon and Washington Pharmacy License and Pharm.D.
• ASHP-accredited residency in Clinical Pharmacy Practice or equivalent background/experience.
• Board certification in pharmacy specialties preferred (i.e., Ambulatory Care-BCACP, Pharmacotherapy-BCPS).
• 3+ years of experience in clinical pharmacy practice including clinical pharmacotherapies, disease state management, and drug use evaluations, or completion of 1+ years of accredited pharmacy residency program.
