Remote LVN/LPN Care Management Opportunity
We are seeking a compassionate and experienced LVN/LPN to join our Care Management team in a remote capacity. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care to our members, while working collaboratively with healthcare providers and community organizations to address the full continuum of their health care and social determinant needs.
In this exciting opportunity, you will have the chance to make a life-changing impact on our members, who are enrolled in Care Management and present with complex health and social challenges. You will utilize your clinical expertise and excellent communication skills to facilitate seamless transitions to the next care setting, ensure post-discharge goal attainment, and provide clinical assistance to determine appropriate services and supports due to member health needs.
Key Responsibilities:
- Conduct post-discharge calls and complete required assessments to ensure members receive necessary care and support
- Collaborate with healthcare providers, community organizations, and care teams to address member health and social needs
- Provide clinical assistance to determine appropriate services and supports due to member health needs
- Evaluate health and social indicators to identify barriers to achieving optimal member health
- Utilize weekly and daily reporting to identify utilization and reduce Emergency Department Utilization and 30-day hospital readmissions
Requirements:
- 2+ years of LPN or LVN nursing experience
- Active and unrestricted compact LPN or LVN licensure in state of residence
- High School diploma or equivalent; LPN or LVN certification required
Preferred Qualifications:
- Bilingual (Spanish) a plus
- Discharge planning experience
- Advanced proficiency in Microsoft Word, Excel, and Outlook
- Ability to multitask, prioritize, and effectively adapt to a fast-paced changing environment
